If any notice has required to be given to either party under this agreement shall be sent via post or mail to the appropriate authority.
General provisions:
a. PRATIKSHA HOME CARE NURSING warrants that it is adequatly ensured to its employees and others incurring loss or injury as a result of the acts of the NGO or its employees.
b. All work shall be completed in a professional manner and it is applicable in complaince all applicable laws.
c. In the event, if client has fail to pay any periodic or installment payment due hereunder, PRATIKSHA HOME CARE NURSING may cease work without breack pending payment or resolution of dispute.
d.PRATIKSHA HOME CARE NURSING shall not be liable for any delay due to circumstances beyond its control.
e. PRATIKSHA HOME CARE NURSING is an independent service provider agency and not an employee of the client.
f. Neither the client nor the agency may assign the agreement without prior consent from the both parties along with fill up the online registration form by the client.
g. If any changes has to be added on the agreement the documents must be signed by both parties.
h. The headings of the sections herein are for convenience only and shall not affect the meaning of the provisions of this agreement.
i. This agreement shall be constructed in accordance with the laws of India.
j. This agreement constitute the entire agreement between the client and PRATIKSHA HOME CARE NURSING and supersedes any prior understanding or representation of any kind preceding the date of this agreement. There are no other agreements whether oral or written relating to the subject matter of this agreement.
Additional Clauses:
In withness whereof the parties have executed this agreement on the date of first week of the month which has be clearly mentioned above.
Client signature
- Client Shall has to be paid to PRATIKSHA HOME CARE NURSING a sum of 1500 (one thousand five hundred) repees as registration fee.(non refundable)
- Client shall has to pay in advanced first month payment as security deposit after arrival of the nursing assistant/caretaker/babysitter to the PRATIKSHA HOME CARE Account a sum of rupees................................... for the services.
- Client shall also has to pay the salary of the man in services of PRATIKSHA HOME CARE NURSING for the services a sum of rupees.............................. within 5 days of after the completion of every month.

PRATIKSHA HOME CARE NURSING
PROTOCOLS:
1. The agency provides housekeeping/ nurse/ caretaker staffs to commercial and residential client by making an agreement and taking a security deposit amount.
2. The daily shift consist of 24 hours.
3. The registration fees of Rs 1500 (one thousand five hundred) has to be paid by the client, which has to renewed after one year (non refundable).
4.The agency provides nurse/housekeeping/caretaker staff after complete documents verification.
5. The client has to provide ID and address proof documents along with 2 copy passport size photos, a copy of legal agreement document for official purpose.
PRATIKSHA HOME CARE NURSING
PROTOCOLS:

6. The photocopy documents of personal details of man power in services will be provided to the client if required.
7. There will be 2 leaves in a month for the nurse/house keeping/ care taker)

9. If required the agency will provide an attendance sheet against the manpower in services, which will be collected by the field supervisor from the household at the end of every month.
10. The client can terminates or discontinue the service but has to inform before 10 days in written or mail to the respective authority of the agency and if not so then the agency will never be refunded the security deposit.

11. If agency unable to continue the service or the client does not want to continue the service on genuine gtounds then it takes 3 Days to refund money to the client with 80% cash of remaining services days.
12. If the client changes the service schedule for 3 times without any genuine reason the agreement will be cancelled.
13. Due to the cancellation Rs 1500 will be charged. Whereas at First month of agreement if the client cancelled the agreement without taking replacement of caretaker or nurse the security deposit amount will never be refunded. On the other hand if the patient expire at the first month of agreement then mere only 60% amount from remaining days from the advance security deposit will be refunded to the client.
14. The client has to pay the salary in advance as security diposit for the first month which is compulsory.
15. For any financial transaction agency prefers cashless transaction/ UPI Payment/ Internet Banking.

Whereas the client should refrain from and strictly restricted giving ATM card to the caretaker staff in service to withdrwa Money or keys of home locker (Tizori) to manage money, marketing or any financial matter, if done it by the client without informing the agency will not be responsibble for any unwanted situation.
18. During the services hours, if the housekeeping/nurse/ caretaker meet with an accident the client will be answerable and responsible.
19. If the client wants to change the housekeeping/nursing staff without any proper reason the agency will need at least 7 days to arrange the service.

26. The Pratiksha Home Care Nursing office remains always open and we provide service as 24X7 and try our best to satisfied the clients.
27. Client should not forced the agency to contineu a staff permanently as their preference whereas the agency has a rule of routation of staff, in a particular cause continuation may be also there but it may depands on the situation only.
